how does previous and next patch work?

hi folks, I’m trying to set up foot controller paddles to browse through the patches and I’m coming to the realization that I must not understand how the previous and next patch functions work. Do they search particular folders? Or is it The program change list? I guess I just don’t understand how the patches are structured as files. Would really appreciate if someone could explain this to me because I’m only having two patches come up when I use previous and next patch actions. It doesn’t just seem to naturally cycle through all available patches it must have some other way of choosing patches to switch between.

Also noticed that sometimes patches are listed in the program list but the name of the patch just doesn't come up? Kinda weird. Maybe this is good clue about how it works?
